Don't Forget

It is very important for patients to remember to bring the following items to their first and/or subsequent visits:

• Completed paperwork
• Insurance card
• Type of payment (co-payment – OB payment)
• PCP referral

One important policy with our office is; if you are more than 15 minutes late (for any reason), we will reschedule your appointment.  HWH strives to maintain an on-time schedule for all patients.

Insurance

Should your insurance change, please make sure you notify the receptionist at check in.  We will scan your new insurance card and obtain new benefits.
